If you would like to make a monetary donation, please contact Deana Combs at 859-525-1128, ext. 230 or email her at dcombs@csadultday.org.  All donations are tax deductible to the extent of the allowable amount.

If you wish to donate gift cards (WalMart, restaurants, etc), supplies and/or items to be used for and by the individuals we provide services for in the Adult Day Care, please contact Lisa West at 859-525-1128, ext. 239 or email her at lwest@csadultday.orga.  Gift in kind donation forms will be provided for all items donated.
